Which rotary cutter size should I start with?

A 45mm cutter. It is the size most quilting instructions assume, it cuts four layers of quilting cotton comfortably, and replacement blades for it are the cheapest and easiest to find. Add a 28mm later if you start cutting curves.

Can I use any brand of blade in this cutter?

Yes, in nearly every case, as long as the diameter matches. A 45mm cutter takes 45mm blades from any of the main makers. Decorative blades such as wave and pinking are the exception, and they list the cutters they fit.
